Here are the forms for Cedar point.

GREAT news from Kroger’s:  Once again this year we will be suspending the re-enrollment process. This means members will NOT need to re-enroll their Kroger Plus card to continue to earn for the charity they have chosen.

Two things to note
· If for any reason a member does re-enroll they will just continue to earn as they were.
· As always new members will be able to join the program at any time

Banquet ticket form

Band Banquet Tickets are available for purchase now until Monday, May 1st. The banquet is open to all band students, all grades. Come say goodbye to the seniors and this is where your student receives their band letter or pin if they qualify for one. By attending the banquet you also receive a copy of the 2017-2018 band yearbook showcasing all students- this is not a seniors only book! The Banquet is Monday, May 14th, 2018.  Doors open at 6pm. Tickets can be purchased at the April booster meeting or by mail with a self-addressed stamped envelope. April is the only booster meeting they will be sold at this year! The cost per ticket is $22 per person.  Invite family and friends to attend the banquet and congratulate your student for an awesome 2017-18 year. Student tickets can be purchased from their student account, but ONLY the students tickets, not any one else’s.  Yearbooks are limited one copy per family. Additional copies can be pre-ordered for $5 each before the deadline.

Reminder all Flower orders are due tomorrow at the membership meeting 7pm.

A printed flyer must be presented when ordering or brought with when paying for TO GO.

A printed flyer or an electronic version of the flyer on a mobile phone or tablet must be presented when ordering. The flyers cannot be passed out onsite the night of the event—you must do this ahead of time.
